Membership Addendum For Canadian Applicants

To be submitted together with signed membership application.

Please note:

The membership fee may be paid by check drawn in Canadian dollars. If the official currency exchange rate at the time the check is drawn places the value of the Canadian dollar in the range of US$0.90 to US$1.10 then the Canadian dollar fee amount shall be the same as if paid in US dollars (for example, if the applicable fee is $950 then $950 Canadian will be accepted.) If the exchange rate falls outside of such range, then the membership fee amount shall be converted accordingly.
