DUPLICATE MEMBERSHIP CERTIFICATES

Duplicate membership certificates are available to members.  There is a $200 preparation charge per certificate.  To obtain duplicate certificates, please send a request to the email address below.  Upon receipt of your request an invoice will be generated and emailed to you for online payment.  Upon payment of the online invoice, certificate preparation will be ordered and you should receive your certificate(s) within several weeks.

• current mailing address
• type of certificate(s) - Million Dollar Advocates Forum and/or Multi-Million Dollar Advocates Forum
• number of certificates requested ($200 each)
