Application #1.  MILLION DOLLAR ADVOCATES FORUM  - Application to be a new member of the Million Dollar Advocates Forum.  One-time fee of $1500 for Certification and Life Membership if approved. All fees refunded if not approved.  No annual or other dues or fees to maintain membership. (Total charge: $1500).

Application #2.  MILLION DOLLAR ADVOCATES FORUM with Expanded Directory Option - Same as Application #1 above ($1500 one-time Certification and Life Membership fee if approved), plus the optional Expanded Directory Listing for one year (inclusion of Firm Name, active link to Firm Website and statement of Areas of Practice) - $500 per year. Member may elect to renew Expanded Directory Listing after one year or allow it to expire. (Total charge $2000).

Application #3.  MULTI-MILLION DOLLAR ADVOCATES FORUM  - Application to become a member of the Multi-Million Dollar Advocates Forum.  Note: to become a member of the Multi-Million Dollar Advocates Forum, you must either already be a member of the Million Dollar Advocates Forum or apply concurrently for membership in both by using this form. If you are already a member of the Million Dollar Advocates Forum, the one-time fee for Certification and Life Membership is $1500. If you are not presently a member (and are applying concurrently for both memberships) the one-time fee is $2000. (Total charge $1500 or $2000).

Application #4.  MULTI-MILLION DOLLAR ADVOCATES FORUM with Expanded Directory Option - Same as Application #3 above ($1500 or $2000 one-time fee), plus the optional Expanded Directory Listing  (inclusion of Firm Name, active link to Firm Website and statement of Areas of Practice) - $500 per year.
